I was watching the history of the actions taken from a moderator on Drupal Answers, and noticed the following entries:

All the questions are said to be edited, but that is not true: The questions have been created from the OP, and nobody else edited it. It could be the OP edited the question in the grace period, but Clive (that moderator) didn't edit it.

In all those cases, the question was closed by googletorp. (Clive was not moderator when he voted to close those questions.) If the edit is referring to the one adding the link to the duplicated question, then that action should be reported for googletorp (if such action needs to be attributed to any moderator). Editing a question to add the link is done from the Community user for every duplicate question, and reporting it as done from a moderator would be giving wrong information.
Update
To make it clear, those questions involve two moderators: Clive (who who was not moderator when he voted to close the questions), and googletorp (who was already moderator when he voted to close the questions). The screenshots I took show Clive's moderation history.

googletorp's moderation history (who effectively closed the question) doesn't have an entry for those questions. It means the action of closing the question is reported for the wrong moderator.
googletorp is surely who closed the question. If Clive voted to close the question as non moderator, and then closed the question as moderator, his name would appear twice, as it happened with Which modules should I use?

What happens is that, instead of the closed question being shown in the history of the last moderator who voted to close, the question is shown for the first moderator who voted to close. The last moderator who voted is not the first moderator who voted, when there is a moderator that voted to close a question before to be elected moderator.
Showing the closed question in the history for the moderator that cast the last closing vote works in every case. If there only one moderator involved, the first moderator who voted is also the last moderator to vote; if there are more than one involved moderator, it's the last moderator who voted to close the question.
